Social Worker

A leading nonprofit organization is looking Social Worker to serve the New York area. The ideal candidate would have experience working with the elderly population. The candidate would have the desire and ability to work collaboratively and engage in the improvement of each individual’s life.

Must Haves:

• Masters in Social Work MSW OR bilingual in English and Korean, Chinese, or Russian
• 2+ years experience providing Social Work guidance, ideally from home care programs (case manager, social worker, etc)

Preferred:

• Bilingual in English and Korean, Chinese, or Russian preferred.

Principal Responsibilities:

1. Conduct intakes, provide accurate information and referrals.
2. Screen and assist clients with applications and re-certifications for entitlements and benefits
3. Assess clients in their home, hospital or other institutional setting as appropriate
4. Advocate on behalf of clients and arrange for provision of services; develop implement and follow up on care plans
5. Provide supportive counseling to clients as needed
6. Maintain ongoing communication with client, family members and other collaterals as appropriate
7. Maintain complete and accurate case notes utilizing the program’s data system; prepare required documentation in case file within organizational time frames
8. Maintain and report on statistics according to standards, utilizing the program’s data system and other forms of record keeping as needed.
9. Actively participate in supervision and staff meetings and share any emergent issues with supervisor in a timely manner
10. Facilitate and assist at social, recreational and other group activities and events
11. Supervise social work students (if applicable)
