Hybrid Remote & Field – RN – Bilingual Registered Nurse

Compass Healthcare Consulting & Placement is conducting a search for Hybrid Remote and Field RN Care Manager for a Managed Care Plan servicing New York. The RN Care Manager position works as a Partial Remote Case Manager, working primarily from home, with up to 2 days of Field Case Management visits per week, in designated area. Bilingual Spanish required for Queens Opening! Opportunities in Brooklyn for Bilingual Spanish, Russian or Chinese! Qualified candidates will be able to work from their homes and will make an occasional Field visit, approx. 2 days per week in field. Qualified candidates will have prior RN Case Management experience within Managed Care, Home Care or Hospital Case Management, bilingual language ability a plus. Qualified candidates will have a NYS RN License or NYS NP, and prior RN Case Management experience. Hybrid Remote & Field - RN Care Manager Responsibilities: Reporting to the Supervisor of Care Management, this position will provide telephonic and in-person case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member's care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member's needs.managed care nurse, managed care rn, nursing, rn, nurse, care management Qualifications: * NYS RN license required * CCM preferred * At least 2+ years of RN Case Management experience in managed care, home care, acute care, hospital or nursing home preferred * Ability to make patient visits up to 2 days per week, ability to work remote from home the remaining days * Reliable transportation * Covid19-Vaccination * Bilingual Spanish, required for Queens Opening! * Bilingual Spanish, Russian, or Chinese, required for Brooklyn Opening! * Additional openings available for Brooklyn, NY visits. * Prior experience in an interdisciplinary service delivery environment preferred. * Working knowledge of Medicare and Medicaid regulations Excellent Salary based on relative experience and great Benefits!
