Designated Coordinator
A local non-profit in-home care services agency is looking for an experienced Designated Coordinator to administer admission and supervision of our 245D waivered services.
Responsibilities include:
- Completing screenings/intakes for new clients
- Developing and/or writing and implementation of each client’s person-centered support plans including (CSSPA) Coordinated Service and Support Plan Addendum, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan.
- Staying in compliance with 245-D licensing rules and regulations.
- Performing quarterly, semi-annual and annual reviews as needed or directed by the person served and/or team in order to person-centered services.
Qualifications:
Bachelor's degree in a field related to human servicesAND1 year full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older
- ORan Associate's degree in a field related to human services or public relationsAND2 years of full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older
- ORa diploma in a field related to human services from an accredited post-secondary institutionAND3 years of full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older
- ORa minimum of 50 hours education and training related to human services and disabilitiesAND4 years of full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older under the supervision of a person who meets any of the qualifications above
Bi-lingual in Russian preferred
At least 2 years of previous hands-on experience as a 245D designated coordinator managing IHS/ICLS services required.
